Legion Health is an "AI-native" online psychiatry practice YC holds up as a paradigm case. What it does couldn't be more ordinary in traditional healthcare — psychiatric consults, prescriptions, scheduling, insurance billing. What's unusual is that its growth curve and its headcount curve have completely decoupled: patient count 4x'd over the past year, it serves thousands of patients a month, and dozens of doctors work on the platform — yet every operations function in the whole company is staffed by exactly one person: 1 clinical lead, 1 patient support, 1 billing. Four times the scale, and ops headcount didn't move an inch. In the YC video, the founder credits this explicitly to one thing — not some sexy-sounding "AI does the diagnosing" capability, but an internal console so plain it's easy to overlook: a single source of truth. It pulls everything about a patient, normally scattered all over the place — history, available appointment slots, insurance codes, past messages — onto one screen. In traditional healthcare this information sits across different departments and different systems, stitched together by humans passing messages back and forth: slow, and easy to drop a patient message. Legion compresses it onto one screen, so work that should require an entire team gets carried by one person plus a bit of automation. So what this case is really about isn't a downloadable healthcare product. It's an organizational and process model: build the internal single source of truth first, so ops headcount stays flat as scale grows.
